CANCELLATION POLICY
We schedule our appointment so that each patient receives the right amount of time to be seen by our dentists and hygienists. That’s why it is very important
that you keep your scheduled appointment with us, and arrive on time.
If you do not cancel or reschedule your appointment with at least 24 hours’ notice, we may assess a $75 “no-show” service charge directly to your
account. This “no-show charge” is not reimbursable by your insurance company. You will be billed directly for it.
PAYMENT OPTIONS
There are several ways. One is to use your personal credit card and pay as you would on any purchase you make- Visa, MasterCard, AMEX, and
Discover. Or, you can use a no interest card that can be used only for health care- FSA (flexible spending account) or HSA (health savings account).
DENTAL INSURANCE DISCLOSURE
As a courtesy to you, we will bill your insurance company and track claims. We accept all major insurance plans as long as you can see an
“Out of Network” provider. Most dental plans cover only certain treatments or limit the amount they’ll
pay for different procedures. Many will not pay for any cosmetic services, like tooth whitening. Most limit payment to $1,000-$1,500 per year.
Even though that sounds like a lot of money, with your deductible and copayment requirements, most plans do not pay out their maximum benefits.
Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance
coverage may be. Most insurance companies should respond to the claim within two to eight weeks. Any remaining cost is your responsibility.
Payment is due at the time of service for patients without insurance & estimated copayment for major treatments.
